Diabetes Center Programs
Diabetes Self-Management Program
A combination of group classes and individual consultations for people with diabetes or pre-diabetes. This program provides instruction in all aspects of self-management including self-monitoring of blood glucose, meal planning, guidelines for physical activity, medications and strategies to prevent complications. New classes start weekly.
Optimizing Insulin Management
This program is designed for people using insulin therapy. It provides instruction in insulin preparation, administration, carbohydrate counting and making insulin adjustments for meals and exercise in a safe manner that facilitates optimum blood glucose control and lifestyle flexibility.
Continuous Glucose Testing and Insulin Pump Training
Training and initiation of Continuous Subcutaneous Insulin Infusion is provided.
Support Groups
Free monthly support groups for people with diabetes. The group meets on the 1st Monday of every month, from 7:30 - 9:00 p.m.

Physician Referrals and Insurance Coverage
Your physician must refer you to program classes and individual education. You can download our Physician Referral Form (PDF, 141k)
and have your physician complete the form.
Most insurance plans cover all or most of the program fees. Family members are welcome to attend all educational programs for free. Valet parking is provided at no cost to you at our facility.
