Volunteer Health Clearance Requirement

• TB Baseline - 1 TB Shot is Required of Each Volunteer
• Measles (Rubeola) + Mumps + Rubella
• Chickenpox (Varicella)
• Tdap (Pertussis) Vaccine (Since 2005)
• Hepatitis B Immunity
• 'Flu' Vaccine
• Note: Annual TB Surveillance & Flu Vaccine is required

TB Baseline - 1 TB Shot is Required of Each Volunteer

• TB Shot 1 – at least within the past 12 months
• If TB Shot is positive, a chest X-ray is required.

Measles (Rubeola) + Mumps + Rubella

• Documentation of 2 vaccines for Measles, Mumps, Rubella (MMR)
  
  OR:
• (+) titer for Measles, (+) titer Mumps and (+) titer Rubella (meaning that they have antibodies to each of these diseases

• Documentation of 2 Varicella (Varivax) vaccines
• OR:
  (+) titer for Varicella

Tdap (Pertussis) Vaccine (Since 2005)

• Mandatory if working in Women and Infants, or Emergency Room, must have vaccine
• Must show that the vaccine has been 'offered' but may document 'declination' of vaccine with provider

• Documentation of a (+) Hep B antibody titer (HbsAb)

'Flu' Vaccine

• Documentation of annual flu vaccine or flu declination form required.
• (may be required to wear a mask when in patient care areas if vaccine is declined)
